Terms and Conditions

*Completed prescription or work order (our stationary or yours) will be considered consent to complete work, acknowledgement of your acceptance of these terms and conditions and your agreement to be responsible for payment of applicable charges.
*Any and all case work received from your office must include a completed prescription or work order before any work can begin.
* All invoices are due within 30 days of invoice date.
*Should you choose to receive monthly statements, the date due will be the 15th of the next month. (ie. statement dated May 31 is due June 15)
*Balances carried 30 days beyond due date (invoice due or statement due date) may incur additional fees or late charges and will cause a hold on your account until payment arrangements are made.
